The UCLA Brain Tumor Center has a Care Coordination Team designed to help patients and families manage the complexity of diagnosis, treatment, and follow-up across different settings.
The team includes nurse navigators and patient care coordinators.
The nurse navigator handles the clinical and educational sides of care—answering medical questions, explaining treatments, ensuring the patient sees the right doctors.
The patient care coordinator focuses on logistical and administrative support (e.g. scheduling appointments, coordinating between outpatient and inpatient departments).
Together, they work to ensure continuity and seamless communication among all departments, reduce patient burden, and help connect patients with internal UCLA resources and community support.
